We love to welcome new patients to our office!

So that we may best serve your needs, please register or login to our online portal to fill out your patient forms.   We are happy to help you with this. Please also be sure to sign our Notice of Privacy Practices which states that the information you give us about any insurance coverage you have as well as your general health and personal expectations is kept confidential and is only released with your permission.

If you are visiting us to get out of pain or for a specific purpose, then that will be our first priority.  With your permission, we will painlessly gather clinical data.  This may include digital radiographs and various dental tests.

If you are visiting us to establish a new dental home, then Dr. Levy or Dr. Eldridge first will take the opportunity to sit down with you and discuss your personal needs and expectations. Please bring with you any diagnostic records (radiographs, photos, models, etc.) from your previous dentist.

Dr. Levy or Dr. Eldridge will examine your oral condition with the aid of a video enhanced operating microscope.  Our comprehensive examination includes a survey for evidence of oral tumors or cancer, gum disease, cavities, dry mouth and TMJ or bite problems.  We will discuss how well your teeth function and any cosmetic changes you might want to consider.

Our goal is to give you the best information possible in order for you to make the best decisions to meet your expectations.  This includes various treatment and payment options. Our entire team is dedicated to accommodating your needs and expectations.
