Introducing: The Levy Dental Benefit Plan
Purpose
Provide an alternative to existing individual dental insurance plans at an exceptional value, for preventive services and conservative care with no exclusions, no denials, no deductibles, no benefit waiting periods and no annual maximum limits.
Objective
Help the Levy Dental Group patients achieve oral health and comfort with an emphasis on preventive and minimally invasive care.
Schedule of Dental Benefits
The following preventive services are 100% covered during each 12 month contracted period:
- 2 semi-annual periodic oral examinations
- 2 semi-annual routine dental cleanings
- 2 semi-annual topical fluoride applications
- All necessary diagnostic Bite-Wing, Periapical and Panoramic radiographs (no limits or exclusions)
- 2 preventive kits, including tooth brush, tooth paste and dental floss provided semi-annually
Benefit Discounts
The Levy Dental Benefit Plan offers a 20% discount for the following necessary dental treatments including:
- Fillings (tooth colored or silver)
- Sealants
- Crowns
- Fixed bridges
- Removable bridges and dentures
- Implants
- Root canal therapy
- Deep cleaning for gum disease
- Surgical and non-surgical extractions
- Additional routine dental cleanings
Payment Options
The Levy Dental Benefit Plan is a 12 month contract. We offer two payment options:
Option 1: One Annual Payment (SAVE 5%)
Monthly Cost: $37 / Month
Total Cost: $444 / year
Payment Amount: $444
Option 2:Two Semi-Annual Payments
Monthly Cost: $39 / Month
Total Cost: $468 / year
Payment Amount: 2x $234

Enrollees who maintain the following are eligible to save an additional 5% on the Levy Dental Benefit Plan (regardless of payment option):
- Healthy gums (gingival health)
- Healthy teeth (no cavities or fractures)
- Healthy diet (minimal sugar and junk food/drink)
- Healthy home habits (floss, brush, irrigation as recommended)
- Protection of teeth from excessive force and wear (night guards and athletic guards as recommended)
- And require no intervention as determined by the Levy Dental Group dentist’s diagnosis, during 4 consecutive semi-annual dental cleanings and examinations.
The discount will continue as long as Enrollee qualifies.
